Stress and unresolved emotions.
Continuing care provides a place to bring stress, difficult emotions and emerging concerns as they arise following discharge.
Completing residential treatment isn't the finish line. For many people, the weeks following discharge can be a particularly important and challenging period as they adjust to life outside the structure of residential care.
The transition from residential treatment back into everyday life can bring new challenges. The structure and support of residential care change as someone returns to their usual routines, relationships, responsibilities and environment.
Continuing care helps provide appropriate professional support during this transition and gives clients a planned pathway for ongoing therapeutic and recovery needs after residential treatment.
Continuing care is available to clients completing residential treatment at Samarpan and is planned according to each person's ongoing clinical and recovery needs. Depending on the individual care plan, this may include individual therapy, family support or therapy, group sessions and other appropriate recovery support.
Continuing care planning begins before discharge. The clinical team works with each client to identify their ongoing therapeutic and recovery needs, potential challenges following discharge, sources of support and the level of continuing professional care that may be appropriate.
The transition after residential treatment can involve practical, emotional and social challenges. Continuing care provides an opportunity to address these challenges with appropriate professional support.

Ongoing contact allows the clinical team and client to review how things are progressing and identify when additional support may be appropriate.
Continuing care can help clients maintain connection with professional and personal sources of support during the transition following residential treatment.
As part of Samarpan's continuing-care programme, clients receive a number of complimentary individual therapy sessions following residential treatment. The number of included sessions varies according to the length of the residential programme and accommodation package. These sessions form part of the transition from residential treatment; they do not limit the amount or duration of continuing care that may be clinically recommended. Your clinical team will discuss your individual continuing-care plan with you before discharge.
